HIPPA Notice of Privacy

McNames Mental Health (“we,” “our,” or “us”) is committed to protecting your Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and applicable state laws.


1. Our Responsibilities Under HIPAA

  • Maintain the privacy and security of your PHI

We may use or share your PHI for the following purposes without additional written authorization:

To provide, coordinate, or manage your care, including consultations or referrals to other healthcare providers.

To bill and receive payment from you, your insurance company, or third-party payers.

For activities that improve our services—such as quality assessment, staff training, accreditation, auditing, or compliance reviews.

As Required by Law

We may disclose PHI when required by federal or state law, including:

  • Public health reporting

  • Oversight agencies

  • Worker’s compensation

  • Situations involving threats to health or safety

We only disclose the minimum necessary information for these purposes.


3. Uses and Disclosures Requiring Your Authorization

We will not use or disclose your PHI for the following without your explicit written permission:

  • Sale of your health information

  • Psychotherapy notes (if applicable)

  • Any use not described in this Notice

You may revoke your authorization at any time in writing.

Request copies of your medical and billing records in paper or electronic format.

Request Corrections

Ask us to correct PHI you believe is incomplete or inaccurate.

Request Confidential Communications

Specify how and where we communicate with you (e.g., alternate phone number or email).

Request Restrictions

Ask us to restrict certain uses or disclosures of your PHI. While we are not required to agree, we will accommodate when possible.

Receive a List of Disclosures

Request an accounting of certain disclosures we have made in the past six years.

5. Telehealth Privacy

We use secure, HIPAA-compliant telehealth technology to protect your privacy. All video sessions are encrypted, and no part of your appointment is recorded or stored without your consent.


6. Safeguarding Your Information

We implement physical, administrative, and technical safeguards, including:

  • Encrypted communication systems

  • Secure electronic health record platforms

  • Role-based access to PHI

  • Password-protected devices

  • Regular security monitoring

Despite these protections, no system can guarantee absolute security. We continually evaluate our safeguards to protect your information.


7. Changes to This Notice

We may update this Notice at any time. Changes apply to all PHI we maintain, and the updated version will be posted on our website with a revised “Last Updated” date.
